Self Pay Rates:
Initial Evaluation - $200
60 Minute Psychotherapy Session - $180

Participation in the sliding fee scale is optional and subject to therapist availability. Requests are reviewed on a case-by-case basis and may require additional documentation.
If you would like to be considered for the sliding fee scale, please note this when you initiate contact with Rebarcak Counseling.

If you are a self pay client at Rebarcak Counseling you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

Insurance:
I am in-network with UnitedHealthCare and Optum insurance carriers. If you are not sure if I am covered under your specific plan, please contact your insurance provider directly.

To check your benefits, the best number to call is the one listed on the back of your member ID card. Please contact UnitedHealthCare or Optum and ask the following questions:
Does my plan cover mental health counseling with Jasey Rebarcak?
Does my insurance plan have a deductible that includes mental health appointments? (A deductible is the amount you will pay out of pocket before your insurance pays all or part of your appointment costs)
Do I have a co-payment amount or co-insurance percentage? (This is the amount you will owe each session)
Does my plan cover behavioral health telehealth sessions?
Please note that you are responsible for co-payments, co-insurance, and/or deductibles as indicated by your individual policy. Payment is expected in full at the end of each session.
Out-of-Network Reimbursement:
If you have an insurance plan through another carrier, such as Wellmark Blue Cross Blue Shield, you may be eligible for out-of-network reimbursement.

To check your Wellmark Blue Cross benefits, you should call the number on the back of your Wellmark ID card. These are questions to ask when calling your insurance company to see if you are eligible for out-of-network benefits:
I am seeking mental health treatment services at the Rebarcak Counseling. Does my plan have any out-of-network coverage for mental health services?
Is there an out-of-network deductible, and if so, what is it?
The billing CPT codes will be:
Initial Evaluation – Billing CPT Code: 90791
Recurring Sessions – Billing CPT Code: 90837 (53-60 minute session) or 90834 (38-52 minute session)
What is the maximum allowable for each of these codes? What percentage of this allowable will my plan pay for each of these codes?
How many mental health sessions does my plan cover per year?
What information/forms do I need to submit in order to receive out-of-network reimbursement?
How and where do I submit reimbursement claims (online, mail)?
Are there any time limitations regarding filing my claims?

I will provide monthly “superbills” (i.e., statements for insurance reimbursement) that clients can use to submit claims to their insurance companies for out-of-network benefits. On the 1st of each month, clients will receive an email notifying them that their superbills/statements for insurance reimbursement are available through the client portal.
